Certificates are only available for sacraments received at Holy Family of Nazareth Church, Irving, Texas.

Anyone 18 years of age or older must fill out the form themselves to request their own sacramental record. For those younger than 18, a parent or legal guardian may submit the request form.

The Sacramental Records office will notify you when the certificate is ready for pickup via email.

Please note that the processing time for a certificate is 2-3 weeks, with a fee of $10.
